Registering With The Surgery
Registering with us is easy, just pop in to the surgery and ask one of the receptionists to help you register. All you will need to do is fill in a GMS1 form, available from reception, and we will do everything else for you. You don't even have to notify your previous surgery.
Catchment Area
We can only accept registrations from patients who live in our catchment area. This area is shown below.
What you will need to know:
- Your full name and any previous names
- Your address
- Town and country of birth
- Postcode and telephone number
- Your previous GP and surgery
- Your previous address
It will also help us if you know your NHS number, however, don't worry if you don't know it.
Temporary Residents
You need to register as a temporary resident if you are in the area and need to see a doctor, but don't live in the area, i.e. you are on holiday or visiting family.
You need to register as a temporary resident so that we can send any medical treatment or notes to your own doctor, which can then be added to your medical record, so it is important that you fill in all the details required on the temporary resident form.
To register as a temporary resident, all you need to do is come into the surgery and explain that you are not a patient but need medication or to see a doctor while you are visiting the area. The receptionist dealing with your requirements will then guide you through the form you will need to fill in.
