Annual Health Reviews

We are in the process of changing our review system. This will affect you is you have any of the following conditions:

- Asthma
- Epilepsy
- Heart Failiure
- CKD
- Diabetes
- Stroke
- Hypertension
- Atrial Fibrilation
- COPD
- CHD
- Dementia
- Mental Health

The idea is for you to come in for your annual review during the same month each year. This should make it easier to remember. We will of course send you a reminder letter. If you require a blood test you will be notified to book your appointment one week before your annual review.

If any further appointments are required the Nurse/Doctor will discuss this with you at the annual review.
