We would like to encourage our patients to give their views about the Practice and how we can improve. Would you be willing to participate in our patient group survey process and be contacted by us periodically for feedback on our services?

To participate you can fill out an online version of the Patient Participation Group form or, if you prefer, you can download the form, and once completed it can be returned to the surgery.
