Data Protection

Personal & Medical Information Handling

In accordance with the Data Protection Act 1998

The Data Protection Act 1998 was bought into force on 1st March 2000 and works in two ways.

It gives individuals certain rights. It also says those who record and use personal information must be open about how the information is used and must follow the eight principles of good information handling.

This document sets out the principles Grosvenor Road Surgery will apply when handling your personal and medical information.

Grosvenor Road Surgery collects and uses personal and medical information in order to carry out it’s duty to it’s patients. We use your information to provide healthcare to our patients in accordance with NHS guidelines.

All personal information is held in accordance to the Data Protection Act 1998.

We will collect and use information only for medical purposes. We will only share this information with other organisations with express permission from the patient unless governed by law, or, under circumstances where the practice believes the patient is at serious risk, or we believe the patient puts other members of public at risk of harm.

We may share information with other NHS and medical associations (such as BUPA etc) under partnership working arrangements. We will share this information only when it is lawful, in accordance with the Data Protection Act and when your rights have been fully consideren.

We take great care to make sure the information we hold about you is up to date, accurate, and when no longer needed, destroyed according to local NHS guidelines and policies.

Any information we hold about you which is not needed, will be shredded in accordance with industry standard shredding guidelines, on the health centre premises.

We use a variety of physical, technical and procedural measures to protect personal information from unauthorised or accidental disclosure, loss or corruption.

Our staff and anyone who handles personal information here at the Health Centre has been trained in confidentiality issues, and is working under legal and contractual obligations to keep your information private and secure. Any member of staff or attached co-workers found to breach the terms set out in the Data Protection Act, or their contractual working agreement with the health centre will be dismissed immediately, and if applicable, further action taken.

You are able to view any information we hold on you, on your request, unless we feel this could be detrimental to your health, or, put other members of the public at risk. A charge may be made and will be payable to the practice as a GP will need to be involved in this process and this is not a contractual service.

You are able to:

         Obtain a permanent copy of your records*
-
         View your records without obtaining a copy.
-
         Have information explained to you where necessary, e.g. medical abbreviations.  

*The GP holds the right to withhold information which could be detrimental to your own health, or put any member of the public at risk.  

If you believe any of the information we hold about you is inaccurate, or misleading, it will be checked and corrected if appropriate.
